Park Gastroenterology Associates understands that any visit to the doctor can be stressful for the patient. We want to make sure that your experience in our Cary, NC office is smooth and stress-free. We have included the following patient forms for your convenience. Please print the forms out and bring them in completed when you come to our office as a patient for the first time.
If your insurance company requires a referral, please obtain this from your PCP (Primary Care Physician) prior to your visit. Please bring any pertinent medical records including lab and x-ray reports from your PCP or have them fax this information to us at (910) 650-6461 prior to your appointment if they haven’t already provided this information to us. If an endoscopic procedure and/or more extensive testing are appropriate, the clinical staff will schedule the procedure/testing for you. A full report of the findings will be forwarded to your PCP.
